In case of error while opening the form, you are requested to download the form, save to your local folder and proceed further as per instructions mentioned below.

"Please courier / post the cancellation form and copy of cancelled cheque, the refund request will be accepted and the process will be initiated after the receipt of these documents. The refund will be as per the refund rules within 15 working days; the fee is refunded through RTGS and the payment / refund advice will be sent by e-mail to the e-mail id given in the cancellation form."
